Fee Schedule 2022

Initial 90 minute visit (CPT code 99205 + 90838 + 99354): $450*

Follow-up visit with 50 minutes of psychotherapy/medical management (CPT codes 99215 + 90838): $350*

Medical management only for 30 minutes (CPT code 99214 + 90833): $200*

Out-of-Office Administrative Time: $75 per 15 minute increment**

Forensic evaluations and testifying: Fee schedule variable. Please call to discuss case and appropriate fees.

Payments

We only accept cash or credit at the time of service. A 3.5% transaction fee will be added for all credit card payment. Although we do not bill insurance providers directly, we will provide invoices, receipts for services and any additional documentation needed to assist your submission for reimbursement to your insurance carrier as an “Out-of-network” healthcare provider. Reimbursement rates are variable between 50-80% depending on your carrier and specific plan. Prior to initial visit we recommend that you contact your insurance provider for exact reimbursement percentages and required documentation. Unfortunately at this time we cannot accept patients on Medicare.

Out-of-Office Administrative Time

**Charges will be incurred if you have a communication with the doctor, or staff, that exceeds 10 minutes. Billing will be in 15 minute increments, at $75 for every 15 minutes. Charges will be applied to the credit card on file. Out-of-Office Administrative Time includes, but is not limited to:

• phone calls or emails regarding patient care
• prior authorizations
• phone consultations or interviews with family or friends
• any other administrative needs not covered during a scheduled visit.

Cancellation Policy

Cancellations less than 48 hours prior to the appointment and missed sessions will be billed in full with payment expected during the subsequent visit.

Emergencies

We are currently not available for emergency services. If you should have a medical emergency, please call 911 or go to your nearest emergency room. If you feel suicidal, please call 911 or the National Suicide Prevention Lifeline at 1-800-SUICIDE
